Yes, that does appear to be an umbilical hernia. First time I’ve seen one that wasn’t on an infant. They’re generally congenital but can be caused by injury.
Anyone with an outty technically have a herniated belly button. Just tiny. I had one that increased after giving birth, and had surgery to fix it, but I never let it get THAT big. It was painful enough while still fairly small. >.<
That’s what it looks like to me, second biggest I’ve ever seen. Dude needs to get that repaired before it strangulates and kills him. (No, seriously, that could mean cut off blood supply, followed by sepsis and death.)
