That painting is thought to be the inspiration for Tenniel’s pictures of The Duchess in “Alice’s Adventure in Wonderland”, although Tenniel got one detail wrong — The Duchess is described in the book as having a pointy chin.
It is a type of caul (made from a wire cage and decorated with jewels to show how rich you are, and lined with embroidered cloth) and a veil. It was one style commonly worn by the Flemish in this period.
That style was well on the wane when the painting was done- though an older woman would be likely to wear older, out-of-date fashions. Matsys painted it in 1513, and the fashion was popular in the middle 1400s.
I’ve attempted to re-create this sort of headdress- they’re not easy to make or wear.

This painting is by Quentin Matsys, and is best known as The Ugly Duchess. Some have thought the painting was a jab at the vanity of aging women who wore coquettish adornment, but it seems this was a real lady who suffered from Paget’s disease:
